Abstract:

Oxidative stress is one of the pathogenetic mechanisms associated with the development of arterial hypertension (AH). Increased formation of reactive oxygen species leads to a decrease in the availability of nitric oxide and, as a consequence, to vasoconstriction, which contributes to the progression of hypertension. Living in unfavorable natural and climatic conditions of the North potentiates the development of the northern ecologically conditioned stress with the manifestation of many diseases, primarily cardiovascular, including arterial hypertension.

Purpose – analysis of indicators of oxidative metabolism in patients with arterial hypertension living in the cities of Salekhard and Khanty-Mansiysk.

111patients with hypertension, employees (44,5±8,2 years old) from among non-indigenous residents of the cities of the Arctic (Salekhard is the capital of the Yamalo-Nenets Autonomous Okrug) were examined: 59, of which 28 (47,5%) men and 31 (52,5%) women and the Arctic zones of the Russian Federation (Khanty-Mansiysk is the capital of Khanty-Mansiysk autonomous okrug): 52, of which 24 (4,.2%) are men and 28 (53,8%) are women who have been living in the North for more than 10 years. Using test kits in the blood serum, indicators of oxidative metabolism were determined – lipid peroxidation (POL): lipid hydroperoxides (GPl) and thiobarbituric acid active products (TBK-AP), as well as parameters of antioxidant protection (AOZ): total antioxidant activity (OAA) and thiol status (TS). The coefficient of oxidative stress was calculated using the formula CBS = GPl x TBK-AP / OAA x TC. The obtained results were compared with physiologically optimal values. In addition to vitamin A, insufficiency of varying degrees of severity in the consumption of the studied micronutrients was established, but a particularly pronounced deficiency was recorded in relation to Se and vitamin D, which did not cover even half of the AF.

The POL is exceeded: GPL at 1,25, TBK-AP at 1,17 times from the upper limit of the norm and low AOZ indicators: OAA – at the lower limit of the reference values, and TC – below them, in most patients their values are low. At the same time, the COS exceeded the upper limit of optimal values by almost 5 times, and in 95% of observations, the upper limit of its physiologically optimal values was exceeded.

In order to increase the effectiveness of hypertension treatment, prevent the development of its complications, improve the quality of life and increase its duration in patients with hypertension, it is necessary to optimize nutrition and intake of micronutrients (vitamins and trace elements) with an antioxidant spectrum of action in the form of vitamin and mineral complexes.
