Abstract:

Treatment of a particular disease involves the choice of effective medical technology. Following the principles of lean manufacturing in healthcare, it is necessary to ensure that losses due to the irrational use of resources are minimized. As part of equipping the physiotherapy room of a polyclinic institution with magnetotherapy equipment, two options were considered. The first option involves a set of magnetotherapy devices that differ in the size of the area of influence, and the second option involves the use of a magnetotherapy device built according to the concept focused on group magnetotherapy. The specified apparatus has a reconfigurable field-forming system formed by a plurality of electromagnetic inductors, where a special code sequence is formed to control each emitter.

To compare the options, an estimate of the effectiveness of using a magnetotherapy apparatus with a reconfigurable field-forming system was calculated. The study was carried out using a simulation model built in accordance with the provisions of the queuing theory. The base indicator of the model was the average time for servicing an application, which was considered a patient receiving a medical service - treatment with a magnetotherapy device.

The potential gain when using the second option is associated with the use of unused inductors, their invariance and the possibility of asynchronous operation of emitters. In addition, the model used an adaptive discipline for servicing applications with an increase in the relative priority of the application for the case of an increase in the time spent in the queue. Also, the algorithm implemented in the model takes into account the type of requests being serviced at the current time, and the amount of free resources, which were inductors.

In the work, simulation modeling was carried out for 50 thousand applications with a fixed treatment time of 20 minutes and an average intensity of receipt of applications of 6 minutes in accordance with Poisson's law. As a result of research, the average waiting time for a patient in a queue is minimal for option 2, when the physiotherapy room is equipped with a magnetotherapy device with a reconfigurable polyforming system. So for option 2, the average waiting time for a patient was 23.5 minutes with an RMS wait of 6.7, and for option 1 it was 35.9 minutes with an RMS wait of 20.4.

The result of the work was an assessment of efficiency, calculated as the relative difference in the patient's waiting time for these options. The estimate was 35%, which, in fact, means a reduction in waiting time by 1.5 times for the variant using a magnetotherapy device with a reconfigurable polyforming system.
