New luminescent assays for quantitation of oxidative stress of patients with anca-associated vasculitis

Abstract:
The article is devoted to the estimation of oxidative stress (OS) level in patients with ANCA-associated systemic vasculitis (AAV). The disease is characterized by blood vessel inflammation. Different kinds of cells (neutrophils, monocytes, T-cells, B-cells) are involved in the pathogenesis of AAV. ANCA-associated activation and increasing production of reactive oxygen species (ROS) by neutrophils play an important role in the damage of the wall of blood vessels. The role of free radicals is not limited to direct damage of molecules. ROS play an important role in the regulation through ROS-signaling pathways, including the activity of T-cells. Knowledge of the oxidative status of patients, on the one hand, may provide new targets for therapeutic treatment and, on the other hand, give new indicators to assess the severity of the condition and the effectiveness of therapy. Oxidative stress in patients with ANCA-associated systemic vasculitis (granulomatosis with polyangiitis, eosinophilic granulomatosis with polyangiitis and microscopic polyangiitis) was assessed by new methods. Methods include chemiluminescence analysis of the radical-producing activity of neutrophils - a source of reactive oxygen species (ROS); fluorescence analysis of oxidized albumin - a target for ROS, and chemiluminescent assay of plasma anti-oxidant activity. Forty patients with AAV were included in the study. Estimation of functional activity of phagocytes was performed by chemiluminescence method using double stimulation with two soluble stimuli with a different mechanism of ac-tion - phorbol-12-myristate-13-acetate and formyl-methionyl-leucyl-phenylalanine. Modification of serum albumin under the action of free radicals could be the index of the depth of systemic oxidative stress. In order to assess OS, fluorescence method was applied. Antioxidant status of blood plasma was estimated by the method of kinetic chemiluminescence (CL). The principle of the method is based on recording kinetic of chemiluminescence and complete analysis of CL curve. The method is based on suppression of CL by plasma antioxidants in the present of luminol (enhancer of CL) and azo-compound (generator of free radicals). Increasing the part of oxidized albumin to 0.48 compare with the control group (0.24) indicates systemic oxidative stress. The depth of oxidative stress depends on the generalization of the process. According to the data, neutrophils are in the primed state. The increment of the basal level of ROS production and the increase response to stimuli were shown for neutrophils of AAV patients. However, attenuation of the respiratory burst occurs faster in neutrophils of patients with ANCA-associated vasculitis, that seems to be connected with immunosuppressive therapy. The antioxidant activity of plasma of patients exceeds antioxidant activity in the control group, which makes dubious the need for therapy with antioxidants. In the future, the authors are going to expand the range of used methods and the group of patients. The authors plan to estimate and compare the level of OS for the different disease activity, generalization process and type of systemic vasculitis in order to develop diagnostic and prognostic criteria.
