Abstract:
The task of decision-making about possibility of an operation of an oncologic ill of old age is re-searched. The decision is based on an estimation of a functional state of the patient when it is ne-cessary to determine, whether it is possible for him to perform radical operation with surgical re-moval of the struck body or the common state of an organism of the patient allows to carry out only palliative operation directed on improvement of a condition of the patient and on elimination of symptoms menacing to a life at impossibility of removal of the struck center. The variant at which the functional state of the patient does not allow to carry out surgical intervention is possible also. In this case it is probably only medicamentous treatment.
The following tasks have been put: to reveal the latent interrelations between initial para-meters and a functional state of the patient (diagnosis); to determine the most significant para-meters for diagnostics; to develop algorithms of decision-making on operating of patients of old age. For the decision of tasks the clinical parameters of 281 patients which past treatment has been analyzed. For each patient the necessary data, the diagnosis put by the doctor, data on a kind of the carried out operation and a state of the patient at an extract have been taken from the case record.
Researches have shown that the following algorithm of decision-making on operating is op-timal. At the first stage it is necessary to reveal, whether the common functional state of the person allows to carry out operational intervention. In a case if operational intervention is impossible, the analysis of attributes stops. Otherwise on the second stage it is necessary to determine which operation can be carried out.
The technique of the step-by-step discriminantal analysis with exhaustion and gradual with-drawal of the least significant attributes was used. It has been revealed, that from 39 attributes now in use in clinical practice, only 15 parameters are significant, the others do not render essen-tial influence on decision-making. In particular, it has been determined, that for initial revealing an opportunity of operational intervention it is necessary to consider an attribute "Duration of impassability" (from group of special oncological attributes) and one of general physiological attributes: - Presence of a heart attack - or "Infringement of a rhythm".
During research the discriminantal functions which were a basis of models for a choice of the most adequate type of treatment of oncological patients have been received.
