Apoptotic mechanisms in the left and right ventricular myocardium in experimental acute coronary insufficiency

Abstract:
According to the results of the investigations presented in numerous papers, chronic heart failure is accompanied by activation of apoptotic mechanisms in the myocardium. With due account for the leading role of atherosclerosis and coronary heart disease in the development of this pathology the works studying cardiomyocyte apoptotic death in acute coronary insufficiency are of great interest. Often different authors do not have the same opinion on this point. In some papers activation of cardiomyocyte apoptosis in the stunned myocardium is put in doubt. However most of the authors consider apoptotic mechanisms to be enhanced in case of ischemic damage to the heart. Some investigations also revealed augmentation of cardiomyocyte apoptosis in the right ventricle (RV) due to focal ischemia of the left ventricle (LV). In the present work acute coronary insufficiency was modeled in rabbits by ligation of the descending branch of the left coronary artery at the boundary between its middle and lower thirds. 1, 3 and 5 days after surgery apoptotic mechanisms in the left and right ventricular myocardium were studied by TUNEL assay and assessment of caspase 3 activity. It was found that apoptotic index significantly increased in the LV on day 3, caspase 3 activity increased on day 1, reached its maximal value on day 3 and by day 5 returned to the control level. In the RV apoptotic index and caspase 3 activity were significantly higher compared with controls at all the investigated terms. Thus acute coronary insufficiency is accompanied by activation of apoptotic mechanism mediated by caspase cascade in the viable area of the LV. Acute focal ischemia of the LV is also followed by enhancement of caspase-dependent mechanisms of programmed cell death in the RV.
